- Confirm that you qualify for the program.
- Call one of the participating veterinarians
- Tell the veterinarian that you qualify under the Maddie’s Fund program, and that you would like to make an appointment. The veterinarian will schedule you as soon as they have an opening. If the veterinarian cannot schedule you at a convenient time, you may call other participating veterinarians.
- When you go to your appointment, bring your 1) Medicaid or Nevada Check-Up documentation, 2) picture ID, and 3) the required co-payment ($20 for a dog, $10 for a cat).
- Follow your veterinarian’s instructions regarding pick-up times for your pet and post-operative care.
- If you arrive at your appointment and the veterinarian feels that your animal is not healthy enough to undergo surgery, the veterinarian may decline to spay or neuter your pet.
- Some veterinarians may place restrictions on the animals they will spay or neuter for this program. These restrictions are noted on the list of participating veterinarians
- Your veterinarian may offer you additional services, such as blood screening, tooth cleaning, or vaccinations. You are NOT required to agree to additional services in order to qualify for the program. If you choose to authorize the veterinarian to perform additional services, you are responsible for covering their cost. The Maddie’s Spay-Neuter Project will pay only for the cost of the spay-neuter surgery.
