- Have a current Medicaid Card OR are a the parent of a child covered by the Nevada Check-Up Program.
- Reside in one of the following counties: Washoe, Carson City, Douglas, Lyon, Churchill, Elko.
- Have dog(s) and / or cat(s) that have not yet been spayed or neutered.
- Both dogs and cats are covered in Washoe, Carson, Douglas, Churchill, and Elko Counties. ONLY DOGS are covered in Lyon County.
- Each qualifying household may have up to six animals spayed or neutered under this program. Additional animals will not be covered by the program.

If you qualify, you may call a participating veterinarian to make an appointment to have you pet spayed or neutered under the program. You will be required to provide a small co-payment to the veterinarian ($20 for a dog, $10 for a cat), but all other costs will be covered by the program.
